I have a lot of information to share with you this time of year and want to focus this letter on recent developments at our agency.
I hope you have had a chance to read the articles by our Senior Care Manager, Amanda Powell, published in the Boomers and Beyond insert. We continue to work hard on our unique Senior Care Management Program. Recently, we attended a national association conference on private duty in Phoenix and brought back enhancements to the program that we feel will be added benefits to our clients and community. We have developed a program to help families with intense needs to manage their care at home. Our staff bridges the communication gap when family members can’t always coordinate the complex care needs and multiple medical reports that come from health care personnel as part of the care delivery system. A recent service of our program has been the addition of a safety assessment and consultation done with the family to determine transition back into the home after a medical recovery in a facility.
Anagency-wide project we have worked on are continued efforts to assist our clients with chronic conditions. This initiative addresses ongoing health concerns, helps prevent any further deterioration in their health and empowers our clients to take an active role in their health care. Our nursing staff is Chronic Care Certified, and a recent enhancement to our program has been a set of care guides given to all of our clients and families to help them troubleshoot when our staff members are not in the home, along with specific measures they should take to maintain their health including fast reaction in calling us for guidance when a change in health happens.
Our agency was an industry pioneer in adopting electronic communication more than 15 years ago. Our advancement in this field continues as we extend electronic communication to all our field staff. This centralized program inputs care data for our review and helps our offices remain efficient so we can continue our focus on client care.
We now have pictures to share with you from our son’s wedding this past July. Dan and Missy had a beautiful ceremony. I hope you enjoy the pictures as much as we enjoyed the event and the welcoming of our new daughter to our family. We are looking forward to spending the holidays with our growing family.
