I receive calls every day from older adults and their families inquiring about the types of services available in the home or the options they have in transitioning to a higher level of care. With the growing aging population, Senior Care Managers like myself are a valuable resource to clients and their family members by offering specialized services for planning and coordinating care of the elderly and others with physical or mental impairments to meet their long-term care needs, improve their quality of life and maintain their independence for as long as possible. My job is to work with clients and families in managing, rendering and referring various types of health care and social services.
Besides helping families navigate the health care continuum, I coach, train, and educate caregivers as well as assess the ongoing safety and well-being of the senior and their family. I can also assist with or attend doctor appointments and relay accurate information at these appointments about medication, diagnosis, and current and past health concerns.
I accomplish this by combining a working knowledge of health and psychology, human development, family dynamics, public and private resources and funding sources, while advocating for clients throughout the health care system. I also integrate health care with other needed services such as: housing, home care, nutritional services, assistance with activities of daily living, socialization programs, as well as financial assessment related to qualifying for services.
Senior Care Management is a service through Western Illinois Home Health Care that has proven to be essential to families, especially working caregivers who live at a distance. Often having a contracted Senior Care Manager in place saves a caregiver or family member from having to take time off work and travel to deal with immediate issues. In most cases the Senior Care Manager can help resolve a crisis because they can be an unbiased arbitrator with the professional background and knowledge of the services needed.
